Residential aged care toolkit
Healthcare professionals and aged care staff look after the many needs of older people as they approach end of life. These roles demand a strong sense of awareness; attention to detail through assessment, documentation and communication; and the ability to provide comfort and support to residents and their families during a difficult time.
Altura Learning has had the pleasure of working with ELDAC (End of Life Directions for Aged Care), to produce a series of educational videos to accompany the Residential Aged Care Toolkit, which focus on five key aspects to improve your practice:
- Recognise End of Life
- Assess Palliaive Care Needs
- Work Together
- Manage Dying
- Organisational Support
The videos bring together clinical leaders, experts and aged care staff to share advice and provide guidance on everything from clinical care to goals of care and advance care planning. They outline ways that organisations can support staff in providing quality palliative care through a coordinated approach and where to find more information in the ELDAC Residential Aged Care Toolkit.
